Before you make your appointment for a COVID vaccine, check your vaccine and booster eligibility. At Health Park Pharmacy, we vaccine those 10 years and older now that the state of emergency has ended.


Book an Appointment for COVID Vaccination

If you are interested in receiving a COVID-19 vaccine or a booster shot, please make an appointment below. Please check you or your child’s eligibility before you make your appointment. Minors must be accompanied by a parent. In order to limit your wait time, please download, print, and fill out the Consent Form below legibly before you arrive. Please bring a copy of your Medicare card if applicable. If you don’t have Medicare, please bring a copy of your insurance card. If you are uninsured, please bring a copy of your drivers license or other form of personal ID.

  • Flu for ages 10 and up

For vaccines other than COVID, there is no need to make an appointment. We accept walk-ins Monday-Friday from 9am to 5pm. If possible, please plan to visit us for vaccines on Mondays, Wednesdays or Fridays from 11am to 3pm when our staff nurse is available. 

FAQs

Q: Can I receive more than one vaccine per visit?
A: You may receive up to 2 vaccines per visit. After that, you must wait two weeks to receive any more vaccines. 

Q: Can we vaccinate pregnant woman for RSV?
A: Yes, if over 18 and between 32-36 weeks. 

Q: Who else is eligible for the RSV vaccine?
A: Anyone 60 years old and older. 

Q: Who can Health Park Pharmacy vaccinate for flu and Covid vaccine?
A: Anyone 10 years old and up.

Q: Who can Health Park Pharmacy vaccinate for all other vaccines provided?
A: Anyone 18 years old and up.
